Two different things are sold as L-carnitine injections, and confusing them is why this topic is so muddled. Levocarnitine injection is an FDA-approved prescription medicine, approved in 1992 for secondary carnitine deficiency from inborn errors of metabolism and for carnitine deficiency in end-stage renal disease patients on haemodialysis. Carnitine is a small water-soluble molecule that the dialysis membrane removes efficiently, so those patients genuinely become depleted, and the injection replaces what dialysis takes out. The other product is the med-spa fat burner or lipotropic shot, usually L-carnitine with vitamin B12 and sometimes methionine, inositol and choline. It is compounded, off-label, not FDA-approved for weight loss, and has no controlled trial evidence behind that claim.
